As the incarcerated population ages, healthcare professionals are increasingly called upon to address the complex needs of individuals with serious illness, cognitive impairment, and terminal conditions within correctional settings. This activity will examine the demographics and health challenges of aging incarcerated populations, explore hospice and palliative care services available within the California Department of Corrections and Rehabilitation, and review the process and benefits of compassionate release. Through real-world examples and practical strategies, participants will learn how interdisciplinary teams can support patient autonomy, advance care planning, family engagement, symptom management, and dignified end-of-life care while navigating the unique ethical, operational, and systemic challenges of the carceral environment. Learners will leave with actionable approaches to improve care delivery and outcomes for justice-involved individuals facing serious illness.
