Thinking skills are important in people with a diagnosis of psychosis as they are associated with poorer outcomes and more dependence on psychiatric services. So treatments that support and improve cognitive skills and metacognition are likely to be helpful in terms of improved recovery. This talk describes the chequered history of developing a treatment to improve cognition – Cognitive Remediation Therapy. It is assumed that treatments, especially psychological ones, are adopted once there is clear evidence of effectiveness. But that is only one part of route to treatment provision. Along the way there are other barriers including the biases of clinical scientists, the acceptance of the outcomes measured, and considerations of what constitutes evidence. Implementation requires clinicians to be aware of the problem and for services to be able to provide the treatment. This talk spans more than a century of knowing there is a clinical problem and trying to fix it.
